They look at specific key bits or terms of sites' URLs and code and make a decision from there. There are frequent issues concerning actual informational websites that could be used as sources to explain something or for a research project being blocked, yet some sketchy stuff passes by the system very easily. Apart from the majorly overt, which sites are blocked each time the systems are updated is kind of a gamble.)
